Cleans up the look of the insert link display. Wraps the form inside
of the standard box that is used everywhere else. Also goes back to
the side by side layout. Can easily be changed to top to bottom if
desired. Most of the changes are formatting as a result of the added

ah well. i had hoped to do away with layout tables, but for some
reason the float layout breaks if an element is loaded later (the
sitetree in this case) - i got sidetracked before i could work out a fix.
but thanks for this commit, it's definitely a usability improvement.
If you want to change it in the future, knock yourself out. I just
thought it could use a bit of cleanup to look like everything else.
