Turns out the problem is AbstractGroup.remove does not save the user's profile.
AbstractGroup.delete does call AbstractGroup.removeAll, but this doesn't work.
To replicate.
1) Select a group
2) Edit members
3) Remove someone
4) Save
5) Kill Lenya
6) Start Lenya
7) Look at removed user, they will still be in the group
