Are there any errors in the log (build/lenya/webapp/WEB-INF/log4j.log)?
Maybe FCKEditor adds some attributes to the <img> element that are not
supported by the schema (e.g., @title). If this is the case, you could
filter these attributes in clean-xhtml.xsl.
