Looks good to me, but I do have one suggestion: Could there be a way to add
more comments as to what is wrong, or a way to reference a trouble ticket
number? Categorizing them is good, but sometimes more information would help.

Even if you just had another text box, it could work something like this: If
the test message was stored as "msg00001-content.txt" then the contents of
the description box could be added to "msg00001-descr.txt". The actual names
would be whatever you'd like, of course, but that's the general idea.
