Yesterday I had the same issue after an update of my PhpMyAdmin installation. They modified the CSS file (some positioned background images) and Firefox showed the wrong icons, even after pressing Ctrl + F5 several times. After restarting Firefox everything was displayed correctly. So that's probably a bug in the latest firefox...
