Here's a patch that addresses most of the stuff.

I'd moved it but it makes sense to move it back (done).
About 2 clients racing to create the same id, I don't think that would happen
(if you're talking about the OCP creating conflicting id) as it's currently
single threaded and at a given time only one task would get processed.
I'll however look at it again to see if I'm missing something.
