I started on SpanNotQuery, see first patch. This is mostly a cleanup to have an 
implementation that is better for a DISI subclass.
This still has asTwoPhaseIterator commented out to make all test cases pass. 
With this asTwoPhaseIterator some test cases fail, I have not yet investigated 
why.

Added, 11 April: I have deleted the patch for SpanNotQuery.
