Updated the patch to trunk with the following:
1. Some bug fixes.
2. Stress test.
3. Some cleanups.

There is one very very rare situation whereby after a commit the recently
reopened searcher doesn't see a dv update which was supposed to be in this
newly opened searcher. There maybe some issue in having the updates flushed
properly at the Lucene level; I have so far been unable to reproduce this
during one off plain Lucene tests or Solr tests, but it is exposed only during
the Stress tests (once in 2000 runs or so). I'll continue to investigate this,
perhaps by writing a similar stress test for plain Lucene level DV updates.
Apart from that, I think this patch is feature complete.
