I've got a little further here and managed to figure out the TOC stuff. So
now we can have 'packages' which we define ourselves. Each 'package' can
consist of one or more projects in the VS solution. When you click on each it
will just display the namespaces found in that package.
I'm stuck with the breadcrumbs but looking better so far and we have
control of the order of 'packages' too. For now I've removed all of the other
doc stuff apart from just the API docs until i have that solved. I've logged an
issue/question on the docfx tracker about the breadcrumb
