Ok I have setup the example, took me a while, I had to port it to the jakarta 
namespace to get it working here: 

I can reproduce it... 4.0.2 und 4.0.1 seem to expose a correct behavior, the 
issue is indeed the issuing item appending. Here is a wild shot, the blur event 
appends also the name key value pair, we only should do that for submit cases 
not for blur or any other event), I guess. This should fix the issue. Maybe I 
can fix this on the client side only!
I will give it a shot at the weekend!
The server side code is correct here, because it does not have to deal with 
special events like blur, in case of non ajax cases!
