Usually you need to provide some information, such as how many committers and PPMC members were added during incubation, how many releases were made, etc. If it is OK, then there is a checklist, about maturity assessment, see this example https://cwiki.apache.org/confluence/pages/viewpage.action?spaceKey=INLONG&title=Maturity+Assessment+for+InLong And also, you need to decide your PMC members after graduation, this discussion could be done in the private mailing list.
