Replacement orders' grand total is set to zero using one header adjustment
instead of order item adjustments.

When a "replacement return" is created a replacement order is also created.
I've noticed that the grand total of the replacement order is set to 0.0 using
one (negative) order header adjustment. Wouldn't be better to create instead
one order item adjustment for each of the order items?
