Implemented functionality for getting list of all orders which were placed by 
customer.
Done following changes
1) Added data in OrderPortletData.xml file and MyPortalTypeData.xml
2) Created form in RequestForms.xml
3) Added a new screen for list of order.

In this default roleTypeId is PLACING_CUSTOMER and statusId is ORDER_APPROVED.
