This issue doesn't persist anymore. Steps that are followed to confirm this:

1) Created a new promo and make it available to use for the current store.
2) Created promo rule with no condition and action as 'X Product for Y Price', 
quantity as 1 and price as 5() Just for example.
3) Added one product as action product in which the promotion should run.
4) Added the particular product into the cart and everything is working fine as 
expected.
5) Also checked by adding another product and adjustments was not being applied 
on second product.
