I created a menu item with a width of 1000 and a height of 400
{code}
        <menu-item name="NewProductionRun" 
title="${uiLabelMap.CreateProductionRun}">
            <link target="nProductionRunLayer" link-type="layered-modal"  
width="1000" height="400"/>
        </menu-item>

This should show a modal window (as per attached image #1) that fits the form.

In stead it shows a modal window (as per attached image #2) where fields run 
outside the window.
