Per Patrick's suggestions, this patch removes the equals() and hashCode()
methods from JDBCFetchConfigurationImpl into the inner cache key class
(SelectKey class) . It also sets fetchGroupContainsDefault to true in
FetchConfigurationImpl.clearFetchGroups().
