[OPENJPA-2996] Exercise the lazy not-loaded path in load state tests
testNotLoadedLazy duplicated testNotLoadedEager in TestPersistenceUnitUtil,
TestPersistenceUtil and TestProviderUtil. Also attach and persist the
RelEntity built by createLazyEntity so testLoadingLazyAttributeByName
really loads a non-empty relation.
