When you put pagespeed off, the resources must have a cache-control header like that you posted (Cache-Control="max-age=604800, public") You can test it whit https://www.mydomain.com/?PageSpeed=off and look at the headers of resources. In the image you have posted, you have debug messages like <!-- Resouce headers are preventing............ these messages are due 2 main reasons, the resouce return a 404, and you need to put the file in their location or delete the html call to these resource, or the resource have a cache-control that is not public cacheable like cache-public="max-age=0" or "private" or "not cache". Whit these changes I spect to get cache-control="max-age=604800,public" in the resource header when pagespeed is off
