I fixed the transformation issue in revision 1236359, so that the current
status is 4 out of 4 ;-)
Any additional tests and samples are welcome.

Thanks for the pointer. The sample I found at pdflib.com helped me finding the
solution as it is way much simplier than the other samples I have.
