While investigating the attached SO question I found that there's some
uncommitted code related to PDFBOX-4080 on my computer: the PDDocument object
isn't passed to the constructor of the annotation handler in PDAnnotationMarkup
and PDAnnotationTextMarkup. This results in the improvement by [~gena77] not
getting used :-(
