I omitted removing the inversion when changing the pixel in my comment :)
For my other PDF and gs-bugzilla692158-schleuse-veryslow.pdf w2/h2 is 0 as
imageTransform.getScaleX() / imageTransform.getScaleY() is 0.0. I suspect that
this is causing problems with those.
