While looking at our test coverage in jacoco output I noticed that this case
was forgotten in tests.
The attached file was created by editing the original PDF and removing all
pages and the content stream and then saving the result. The original file is
from the US dept of health and human services. The annotations are invisible
and their content doesn't really matter anyway.
