Thank you. Re 4, the font stack is relevant when saveGraphicsState() and 
restoreGraphicsState() are called. The handling of fontSizeStack is missing 
there.

For the same reason, I think we also need the same stack handling for the 
glyphLayoutProcessor, if we assume that there could be several processors.
But lets wait, nothing has happened yet and it's not difficult to implement.

Re 6 and the extra comment, we do have this in fontbox and it already works. 
Fontbox has implemented some gsub tables. I could probably implement this but 
after having a look I don't think it's needed, the substitution is done in 
codeToCID() because the cmap is a substitutingcmap, that is done in 
TrueTypeFont.getUnicodeCmapLookup().
