[~rajeshbabu] - should CSV bulk load work on a table with local indexes? How
would the correct region start key prefix get added to the index table rows?
Should we disable CSV bulk load for a table with local indexes? Is it only a
problem when the data table is presplit? Probably not, right, as the data table
will end up splitting as it grows, right?
