[~tdsilva] - would you mind reviewing this one? I'll wait to commit until after
the test run too. I've removed some dead code, ensured that index mutations
don't get generated and send over from the client for deletes involving local
indexes, and fixed the check for determining whether or not we need to scan for
the prior data row.
