CSVBulkLoadTool will generate incorrect index updates for a mutable secondary
index when the records already exist. Specifically, we do not submit the delete
for any previous index records, creating duplicate index records that do not
reflect the reality of the data table.
It's really bad that we don't have this clearly documented.
