org.apache.qpid.transport.Connection maintains a map of channels to sessions in
the channels field, used by map/unmap and getSession. AMQConnection maintains
it's own map of these in its _session field.
It appears that the ints used as the channel id will always be in lock-step,
but that is more by chance than design it seems. There doesn't appear to be any
attempt to reuse or otherwise link the two.
