Broker shutdown process does not wait for Connection Closure

Currently during broker shutdown the virtualhost closes all registered
connections.
However, it doesn't sensibly wait for these connections to have been closed.
A) It keeps sending a close frame until it is removed from the _registry. (It
should only send it once)
B) When the connection is closing it immediately removes its connection from
the _registry before it has finished its processing.
So two things to fix.
1) Vhost close should send 1 close frame per connection and then wait for them
to close
2) The connection should not be removed from the _registry until it is no
longer active.
