DISPATCH-1612 Use browser address and port as the default on the console's
connections page when auto-connect fails and there are no saved connect
settings. Fix problem with displaying connect form after manually disconnecting.

> Automatically fill in the address and port that was used to serve the console
> into the console's connect form

> The console will attempt to connect to the router using the same host and
> port that was used to serve the console.
> If that fails (probably due to missing username/password), the console will
> look for a previously saved address and port. If found, the login page will
> display the saved values. If not found, the login page will use default
> values.
> Instead of using default values (localhost and port 5673), the login page
> should use the host and port that was used to serve the console.
