I personally saw one odd instance during rc1 where a single spam went
through unscanned during a split-second where all spamd children were
dead. I haven't seen this behavior repeat though.

Unless we receive confirmation of a real regression since 3.3.1, I
suspect it is time to cut 3.3.2 and call for votes.

I heard back from that reporter. He determined that it was the kernel OOM killer that killed the spamd parent. He is attempting to figure out if spamassassin itself was to blame for OOM on this 4GB RAM server, or if the cause was unrelated to spamassassin.
