    Minor review updates
    
    - don't override hashcode() and equals() (the overrides break the
    contracts)
    - catch Throwable (else ExceptionUtils.handleThrowable() is a NO-OP)
    - comment typo
    - ensure the trackedStream is returned for multiple getInputStream()
    calls
    - NO-OP known safe case of FileURLConnection
