There is no configuration for this. We figured out it doesn't slow down
performance, so it's always done, no way to disable it.

People can create well structured tables, but they need to turn on
“repeat headings” (Writer), or use print ranges (Calc).
Adding some extra UI for this probably doesn't make sense as long as
people can't declare headings w/o repeating them!
In Writer, it might be a good idea to turn on “heading” and “repeat” by
default. Notes 8 is also handling it like Writer, IBM and other members
in OASIS ODF A11y SC think it's OK that it must be repeating.

Can't be done w/o Writer having a general concept for captions.
OOo only fakes to have captions - in reality it's just image+text in a
frame, and some application logic handling this as a caption under
certain circumstances.
I will ask the Writer if something changed in this area meanwhile.

It is not so easy to extend the existing hyper link tab page w/o bigger
redesign, and we expect bigger UI changes with project "Renaissance".
