Summary: Better handling of unsubscribe feature

At present it's not obvious how to use committers/subscribe.cgi for
unsubscribing: one has to select the mailing list first, then the appropriate
email from the drop-down.
There are use cases for both the following scenarios:
* unsubscribe one or more emails from a particular list (i.e. stop following a
list)
* unsubscribe an e-mail from one or more lists (i.e. stop using an email)
