There are a few reasons not to request an account at the time the secretary
files the icla.
One of these reasons has its own handling: invalid requested id.
But there are other reasons that are lumped into icla-pmc-notified:
* no [[VOTE]RESULT] for the person
* There was no requested account id on the ICLA form or email
The icla-pmc-notified email could be more clear what needs to be done by the
person and/or the PMC when these occur.
And the icla form needs to be modified to allow secretary to choose which of
the three reasons applies.
