Minor issue still exists:
If a user selects the dropdown arrow, but doesn't type anything, the "Select
proxy" combobox is blanked out. At that point, we should _disable_ the Submit
button, because otherwise the error handling after a user hits submit is not
very friendly.
It looks like some JS code to disable Submit when the selection is empty was
added previously, but it's not working currently.
