This is very useful - thank you for persevering with troubleshooting these 
problems.

As far as I'm aware all our testing to date has been with Jetty and Tomcat, so 
it shouldn't be surprising that there are some issues using WAS. If we can fix 
these that would be great; the case-sensitive table names is an obvious issue 
to fix

in any case, I'll create a task to update the README and documentation with 
your steps above so that other users don't need to go through the same 
troubleshooting process.
