Can you tell how did you configured server ? I just run book example on tomcat
5.5.20 with
<Host name="localhost" appBase="webapps"
unpackWARs="false" autoDeploy="true"
xmlValidation="false" xmlNamespaceAware="false">
and everything works fine.
