How this can happen? I.e., orphaned package with one or more co-maintainers.

This is definitively repetitive pattern. I wonder whether we have some process problem?

Why the main admin does not ask co-maintainers first? And if they are not interested in, why they are still listed as co-maintainers?
There're few problems which may cause that:

1) There is (was?) bug in pagure which allows any Fedora package to
orphan any package in the distribution.
2) Some co-maintainers clicked "orphan" button to drop themself from the list
of the maintainers but this button deletes silently main admin.
3) No notification was sent to co-maintainers in case if main admin orphaned the package. In case of (1) or (2) main admin is not being notified (happened to me once and I realized my package was orphaned by email from bugzilla (so if there're no bugs opened - the package may be silently orphaned). I think email to <package>-owners@ at least should fix this issue. 4) Package may be orphaned by the team of maintainers (SIG) if all of them
work in one team (SIG).
5) Main-admin does not have time for Fedora, he doesn't want to spend time to contact other people :(
