Fixed wrong reference counting when DID_FALLBACK is used and an early
in-dialog message like PRACK is received and it doesn't match the dialog
elements when the fallback check is performed. In this case there was a
missing refcount decrementing which resulted in the dialog never reaching
a refcount of 0 and thus it was being kept in memory.
