[pua_dialoginfo] more fixing on dangling branches
This is a continuation of 2bc0e4348a0062b218e49322aff554b063808c42 :
* similar scenario of a dangling branch may happen during a parallel forking.
If branch A dies and B answers, there will be no "terminated" for A; So, we
rely on the TMCB_TRANS_DELETED to do last inspection for dangling branches
* also fix for the newly added TMCB_ON_FAILURE TMCB_TRANS_DELETED callbacks how
the to-tag is fetched - to not take it from t->req as it will be NULL, but take
it from the UAC part of the transaction - each branch has a different to-tag!!!
