If you are really that far off when you start ntpd, it can very easily peg at the 500ppm mark. That 500ppm limit means the clock will drift no more than 0.5ms/s, so to correct for 100ms you'd need 200s for the correction to take place and then you are still left with finding the correct clock drift, which can easily take another hour after such a large disturbance. That's why ntpd usually steps the clock when it starts up the first time, which should bring it into the low single-digit ms range.
