I advice against this, as this is the dutch notation. If the documentation is in english, the date format should be as well. That is as in: 'December 5, 2002'. In order to enforce these guidelines, they should be put online in an obvious place. Also, I think it's a good idea to provide an empty template as a starting point, containing the fields you mentioned, the CVS $Id$ tag and the license notice.
