I like the breakdown in release notes by version, and, within those version notes, the changes per module/application.
It's ellegant becaus eyou canfocus on the last changes, but still go back in history.
Xalan uses this approiach (I think most projects do but I'm not sure).
Seems elegant, and keeps the notes legible.
