Thank you for your work on xindy. Here are my comments on your questions:

Good question. Most likely in the same position with regard to O as CANDRA 
E is sorted with regard to E. How are you handling SHORT E and SHORT O? 
I'll look into this issue and get back to you.

Apte and Monier-Williams sort AUM with O. Same in Hindi dictionaries such

In Molesworth's "Marathi-English Dictionary" (1831), there is the 
following sort order: LLA is shorted after HA. Also, KSSA and JNYA are 
sorted after LLA, not with KA and JA, respectively. TRA, however, is 
sorted with TA. A letter combined with ANUSVARA is sorted by that letter. 
These rules are also followed in Tulpule and Feldhaus's "A Dictionary of 
Old Marathi" (1999).

>From Ruth Laila Schmidt's "A Practical Dictionary of Modern Nepali" 
(1993): CANDRABINDU and ANUSVARA are sorted in the same position. KSSA 
sorted with KA. JNYA sorted with JA, but at the end of the vowel order: 
...JO JAU JNYA. TRA sorted with TA. AVAGRAHA is sorted with the vowel 
letter that follows it: the word A'ILA is sorted first by A then by I.
