Hi Venuste, Which version/build of DHIS 2 are you using?
If you are using the individual records (Tracker), make sure your data elements have: Domain Type = "Patient" and Combination of categories = "default". If you want a drop-down with options then also make sure: Value Type = "Text", Option set = <your selected option set> (not "Please Select"). Also make sure you clear your browser cache completely before trying again. Does that help?
