Hello Terence, All data elements assigned to a data set should appear under the "Data elements" tab. Depending on category combination assigned to a data element, the rows in "Data element" tab could be more.
For those data elements of value type yes/no, you should see three checkboxes (No value, Yes, No) when your custom form is rendered for data entry. --- Thank you, Abyot.
