Description:
A paper size, orientation and document scaling can be set in the page
setup 
dialog.  This sets the page break lines correctly, but these settings
appear to 
be ignored during printing.  (output appears to be portrait at 1.0
scaling).

Also, the setting of a custom paper size in the print dialog results in
assertion
failures and then lockup.
