Absolutely, though I think this is more of a process thing than a ddoc
thing - you could easily enough generate those files in a makefile as
part of the build... I've written a program to do this before, but it
was years ago and probably doesn't work anymore (i based it on the html

Exactly. And TOCs and indexes are generated wildly differently depending on the publishing platform. I don't think ddoc should do that.

PR, please. ;-) This is a lack in content, that no macro system can
make up for, as you said.

I wrote a book, surely that gets me off the hook?! hey i found the time
to write a rhyme, but those kind of conceptual things would take a lot
of hours.... probably comparable to the book itself...

You're soon to find out that paradoxically writing a book enlists you for more, not less, work.
