First off, I want to thank everyone for their help. This is a great list
with great people! I looked at the log (doh.. should have looked there), 
and found the issue:

I'll go ahead add dnscache to the ignore listing. So, if I update my 
summaries to 'success' will this correct my problem? Or do I need to 
re-init my vault image?
