Many ISPs simply disallow this or give a different SMTP AUTH server which 
is dedicated to this purpose.

There's no reason to do that, you can use SMTP AUTH.

I think you are mixing two quite different things. SMTP AUTHing already 
identified users doesn't add much security if any.
